If it had processed, and it had gone through twice, you might explain the problem you had with their system and request that they combine both inquiries and only report them as a single inquiry.</description>

a credit reference usually refers to a company or person you have been making payments to, such as a credit card, bank loan, car loan, etc.  It needs to be a place you have a good credit history with, though.  If you have no credit, then put N/A in the blank.</description>
